Affordable Basic Care (ABC) has been created by Primary Care Physicians to provide affordable access to quality primary care services in a medical home environment.

Our focus is on helping individuals and families who:
• Can’t afford an HMO or comprehensive insurance coverage
• Can’t get insurance because of a preexisting health condition
• Want to take action on promoting good health and prevent illness.
• Have an insurance plan that excludes primary care services

This program provides affordability and access to important health care services in a primary care physician’s office. It offers plenty of accessibility via a large panel of participating primary care physicians from which to choose.

• Medications (Drugs)
• Radiology (X-Rays)
• Lab services not stated as covered
• Any office based surgical procedures not listed
• All services provided by a Hospital
• All services provided by an Urgent Care Center

NOTE: As an extra value added benefit any services or procedures provided by your PCP and not covered under the annual payment, may be provided for an additional fee at a 20% discount. To receive the discount payment is expected at the time services are rendered.

Specialist Physician Services. In the event a PCP recommends that a Member requires health care services provided by a specialty physician, the Member may request names and contact information from ABC. The payment to ABC does not cover any services provided by specialty physicians.
